Different creams and gels combine different pain-relieving ingredients for a greater, synergistic effect. The most effective natural ingredients are arnica, cannabidiol CBD oil, glucosamine, chondroitin, celadrin, comfrey root, capsicum, MSM and Green-lipped mussel extracts. The most effective pharmaceutical pain relief gel are those containing diclofenac, which is a stronger version of ibuprofen.

Chronic pain is a modern day epidemic that affects more than 1.5 billion people worldwide.  It costs nations billions of dollars in lost productivity and medical expenses every year. Debilitating pain effects more individuals than heart disease, cancer and diabetes combined.   Doctors throw drug after drug at these pain syndromes with very little long-term success.  Here are some of the best natural agents to reduce pain.
4. Devil’s Claw (Harpagophytum procumbens) – This ominous sounding herb is actually great for treating numerous health conditions, among them are liver problems and heart burn. It also has anti-inflammatory effects and can reduce pain from arthritis, headaches, and low back discomfort. The University of Maryland Medical Center has published several studies that had great success treating Osteoarthritis with Devil’s Claw.
How it works: When back muscles hurt, the pain is usually caused by inflamed tissue. Cold reduces the swelling and discomfort, says Jason Highsmith, a neurosurgeon in Charleston, South Carolina. As soon as you feel pain, apply cold several times a day, 10 minutes at a time, for about three days. A bag of frozen peas works, or try a cold pack, like an Ace Reusable Cold Compress ($10 at drugstores).
Low and lower back pain can vary from dull pain that develops gradually to sudden, sharp or persistent pain felt below the waist. Unfortunately, almost everyone, at some point during life will experience low back pain that may travel downward into the buttocks and sometimes into one or both lower extremities. The most common cause is muscle strain often related to heavy physical labor, lifting or forceful movement, bending or twisting into awkward positions, or standing in one position too long.

Pycnogenol, like white willow bark, is a nutraceutical material that has been used since ancient times. Pycnogenol is derived from the bark of the maritime pine tree (Pinus maritima) and has been used for more than 2000 years. It has been considered helpful for wound healing, treating scurvy, healing of ulcers, and reducing vascular inflammation. It contains a potent blend of active polyphenols, which includes catechin, taxifolin, procyanidins, and phenolic acids. It is one of the most potent antioxidant compounds currently known.[17,118]
Staying in bed for any prolonged period can make you stiff and increase pain. When you don’t move and bend, you lose muscle strength and flexibility. With bed rest, you lose about 1 percent of your muscle strength each day. And you can lose 20 to 30 percent in a week. It becomes more difficult to return to any activity. As you become weaker and stiffer your recovery takes longer.

Physical therapists often recommend aquatic therapy — including exercises done in warm, therapeutic pools — for back pain. The buoyancy of the water helps alleviate strain on the joints to encourage strengthening and gentle stretching of the muscles. Even floating in warm water can help relax muscles and release tension as well as increase circulation, according to the Arthritis Foundation. With home whirlpool baths, try aiming the jets directly at your sore spots for a soothing underwater massage.
Turmeric can be consumed in a variety of forms, including as a spice added to foods or drinks and encapsulated for those who are not tolerant of the signature spicy taste. Two to three teaspoons of turmeric daily can help to provide therapeutic levels of relief and preventive benefits with little side effects. Note: Turmeric absorbs best when taken in combination with black pepper (approximately 1/4 teaspoon of black pepper for every 1 teaspoon of turmeric). (17)
If you need stronger pain relief, talk to your doctor about a prescription topical NSAID. A prescription topical NSAID is stronger than OTC products. Topical NSAIDs also carry less risk of stomach upset, ulcers, or other problems than oral NSAIDs do. The only prescription topical NSAID currently available is called Voltaren (diclofenac). Talk to your doctor to find out if it’s right for you.
PharmacyTimes.com [Internet]. Fudin J. Should Topical NSAIDs Have Strict Heart Risk Warnings?; 2018 March 10 [cited 18 Jun 12]. Although this article’s title implies concerns about topical NSAID safety, it ends up answering that concern with very reassuring data, and it turns into a piece suggesting that the FDA needs to make it clearer that only oral NSAIDs are of concern, while topical is an extremely safe alternative! “ … all topical vehicles of diclofenac delivery result in only a small fraction of the diclofenac that actually reaches the systemic circulation compared with the oral route.” BACK TO TEXT
Last year, I suffered a severe attack of gouty arthritis. I was crying in pain and tried several pain killers without any effect. Then, I consulted my doctor. He told me to not take any pain killers and prescribed Generic Colchicine 0.5mg no rx instead. And thankfully, the pain started subsiding. I would like to recommend this medicine to all patients suffering from gouty arthritis.
Celebrex, Vioxx, and Bextra quickly became the mainstay for the treatment of chronic pain conditions related to inflammation.[71] Within a few years, an estimated 15–20 million people in the US were using selective COX-2–inhibiting NSAIDs on a long-term basis. These drugs became the most commonly used pharmaceutical agent with more than 70 million NSAID prescriptions written each year and 30 billion over-the-counter NSAID tablets sold annually. It was estimated that 5–10% of the adult population used NSAIDs, and among the elderly (a group at higher risk of nonselective NSAID-induced gastrointestinal complications), the use of these drugs was as high as 15%. The general acceptance of these drugs was due to the perceived lack of serious gastrointestinal side effects that had been associated with the nonselective class of NSAIDs.[26,119]
Massage. Various forms of body work can provide temporary pain relief. You can try full-body Swedish massage for  stress relief and relaxation; deep-tissue massage, which uses pressure and slow strokes on deeper muscle tissue to release knots and relieve tension; or myofascial release, which uses long, stretching strokes to relieve tension around the connective tissue of the muscles.
Warnings: For external use only. Avoid contact with eyes, mucous membranes, and broken or irritated skin. In case of accidental contact, flush with water. Keep out of the reach of children. If irritation occurs, discontinue use immediately. Do not tightly wrap or bandage or use with other topical preparations or heating pads. Consult a doctor if you are pregnant/nursing, under 12 years old, have sensitive skin, your condition persists or worsens, or excessive burning or irritation persists.

Over-the-counter (OTC) pain relievers. Nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen (Advil, Motrin IB, others) or naproxen sodium (Aleve), might relieve acute back pain. Take these medications only as directed by your doctor. Overuse can cause serious side effects. If OTC pain relievers don't relieve your pain, your doctor might suggest prescription NSAIDs.
In summer 2018 I had my first personal experience with dramatic relief from Voltaren Gel (topical diclofenac). It’s not the first time I have found it useful, but it was the first time it was amazing. I’d been having some unexplained knee pain intermittently for several weeks when it kicked up a notch or two and became constant and even started waking me up at night. When I finally remembered to try Voltaren Gel — I’m not sure what took me so long — things were bad enough that it was going to be obvious if it worked. And it was! After many days of constant discomfort ranging from 3-6 on a 10-scale, it just ended: half an hour after applying the stuff, I simply didn’t have that problem anymore. Hallelujah! It’s been weeks since then with no relapse. Although the pain was not terrible, this is actually one of the clearest examples of pain relief from any treatment that I’ve ever enjoyed.
Cool it off. To do a 10-minute ice massage, fill up small paper or foam cups about one third full and freeze them. When ready to use, peel away the top of the cup to expose the ice and gently slide over the painful area. Try to avoid the bony parts of the joint, such as the knee cap and elbow points. Cover the affected area with a plastic wrap before applying the ice to protect the skin, and place a towel underneath to pick up the moisture as the ice melts. You can also use ice cubes wrapped in plastic for smaller areas. Cold packs and wraps applied for 15 to 30 minutes may also relieve sore lower back or shoulders.
Many pharmaceutical pain medications, while effective and useful at times, can be downright dangerous, but there is another solution to your pain problem. “Almost always, if we find pharmaceuticals doing the trick, we’ll find a plant doing the same trick—and doing it more safely,” remarks botanist James A. Duke, PhD, author of The Green Pharmacy Guide to Healing Foods.
The good news regarding back pain is this: Most cases of lower back pain are believed to be due to “mechanical” problems of the musculoskeletal system rather than serious illness or chronic health problems. Abnormalities, weakness, and added stress placed on the bones, joints, ligaments and muscles can all contribute to back problems. It’s been found that the most common causes of low back pain (there are many!) include: (8)
The Inactive ingredients are aloe barbadensis leaf extract, arnica Montana flower extract, Actium lappa root (burdock) extract, boswellia carterii resin extract, calendula officinalis extract, carbomer, camellia sinensis (green tea) leaf extract, camphor, glycerin, ilex paraguariensis leaf extract, isopropyl alcohol, isopropyl myristate, melissa officinalis (lemon balm) leaf extract, silica, tocopheryl (vitamin E) acetate, triethanolamine, water, blue 1, and yellow 5. Ilex is a skin protectant for topical skin barrier gels and lotions designed to protect the skin from harmful irritants such as urine and feces. It usually contains good things such as corn starch, white petroleum jelly, and zinc oxide.
Today, the most common conventional treatments for lower back pain relief are medications, including NSAIDS like aspirin and Tylenol, along with more potent prescription painkillers, such analgesics. These drugs can potentially cause adverse side effects in some patients and commonly don’t solve the underlying causes of lower back pain (such as poor posture, obesity or exercise-related strains). Some medications for back pains have even been tied to complications, such as liver damage or intestinal bleeding, when taken for long periods of time or in high doses.

Prostaglandins act as short-lived localized hormones that can be released by any cell of the body during tissue, chemical, or traumatic injury, and can induce fever, inflammation, and pain, once they are present in the intercellular space. Thromboxanes, which are also hormone activators, can regulate blood vessel tone, platelet aggregation, and clot formation to increase the inflammatory response.[92,82] The inflammatory pathway is a complex biochemical pathway which, once stimulated by injury, leads to the production of these and other inflammatory mediators whose initial effect is pain and tissue destruction, followed by healing and recovery.[34,51] A major component of the inflammatory pathway is called the arachidonic acid pathway because arachidonic acid is immediately released from traumatized cellular membranes. Membrane-based arachidonic acid is transformed into prostaglandins and thromboxanes partly through the enzymatic action of cyclooxygenase (COX)[34,57]. There are two types of COX enzymes, COX-1 and COX-2. Both the enzymes act similarly, but selective inhibition (as accomplished by selective COX-2 inhibiting NSAIDs) can make a difference in terms of side effects.
Just how does acupuncture work? According to traditional Chinese medicine, pain results from blocked energy along the meridians of the body, which are unblocked when acupuncture needles are inserted along these invisible pathways. Acupuncture may also release natural pain-relieving opioids, send signals to the sympathetic nervous system, and release neurochemicals and hormones.
Low back pain and neck pain often involve a substantial amount of muscle pain,22 and muscle pain is not particularly inflammatory by nature. Muscle knots (trigger points) are more like poisoned muscle than injured muscle. Although there’s some anecdotal evidence that taking an anti-inflammatory medication reduces muscle pain, mostly it doesn’t seem to work very well. One of the classic signs of low back pain powered by muscle, for instance, is that ibuprofen doesn’t have much effect!

Turmeric. This spice has been used to relieve arthritis pain and heartburn, and to reduce inflammation. It's unclear how turmeric works against pain or inflammation, but its activity may be due to a chemical called curcumin, which has anti-inflammatory properties. Turmeric is usually safe to use, but high doses or long-term use may cause indigestion. Also, people with gallbladder disease should avoid using turmeric.
